The atrial electrocardiogram as a guide to prognosis after mitral valvotomy.

چکیده

A study has been made of serial electrocardiograms of patients with mitral stenosis in sinus rhythm, whose clinical progress was followed for at least four years after mitral valvotomy. The behaviour of the left atrial P wave after operation has been examined with special reference to its relationship to continued clinical progress. In addition, the form of the P wave in the pre-operative electrocardiogram has been studied in relation to the degree of stenosis as found at operation and also to the severity of pulmonary hypertension. Some of our first patients with mitral stenosis treated by mitral valvotomy, remaining in sinus rhythm post-operatively and followed now for four to eight years afterwards, have required a second valvotomy. At the second operation the valve orifice was found to have again diminished in size and in some it was as small as before. Although these patients appeared to have gained great benefit from the first operation during the first two years, and although the surgeon judged the degree of splitting in most to have been satisfactory, the left atrial electrocardiogram did not improve to the same degree as in other successful patients until after the second more radical operation. Absence of significant improvement in the left atrial electrocardiogram after the first operation suggested inadequate relief of stenosis. In this way the post-operative left atrial P wave has acted as a guide to prognosis in this group of patients in sinus rhythm, and has predicted those likely to require a second mitral valvotomy at a later date.

The apical systolic murmur in mitral stenosis.

The purpose of this work was to determine how reliable a guide an apical systolic murmur can be to the finding at mitral valvotomy of incidental mitral regurgitation complicating dominant mitral stenosis. The history of the systolic murmur in mitral stenosis is a confused one, since general agreement was not reached about the timing of systolic and diastolic murmurs in mitral stenosis for nearl...

Percutaneous transvenous mitral commissurotomy in juvenile mitral stenosis.

OBJECTIVE To determine the efficacy and safety of percutaneous transvenous mitral commissurotomy (PTMC), using multi-track double balloon technique in juvenile mitral stenosis. DESIGN Open non-randomised intervention. SETTING Cardiac catheterisation laboratories of The Mater Hospital, The Nairobi Hospital and Kenyatta National Hospital from 1996 to 2001. Mitral restenosis and mitral regurgitation 1 year after Inoue mitral balloon valvotomy in a population of patients with pliable mitral valve stenosis.

To determine the rate of mitral restenosis and mitral regurgitation increase 1 year after mitral valvotomy using the Inoue balloon catheter, 66 consecutive patients with severe, pliable mitral stenosis had their mitral valve area (MVA) calculated by two-dimensional echocardiography (2DE) and Doppler before, immediately after balloon valvotomy, and at 1-year follow-up. Color Doppler studies were...

Dysphagia due to left atrial enlargment after mitral Starr valve replacement.

Case report A woman of 6o developed increasing dyspnoea from the age of 35 until she was 44, at which time she was found to have mitral stenosis, and a closed mitral valvotomy was performed. Initially she was greatly improved, but her dyspnoea gradually returned and by the time of admission her effort intolerance was much increased and was accompanied by orthopnoea, paroxysmal nocturnal dyspnoe...

Acute mitral regurgitation due to leaflet tear after balloon valvotomy.

Percutaneous mitral balloon valvotomy (PMV) has evolved into an effective method for the treatment of patients with symptomatic mitral stenosis. An increase in mitral regurgitation can occur in '45% of patients undergoing PMV. Severe mitral regurgitation can be caused by rupture of chordae or of a papillary muscle. Noncommissural tearing of the mitral leaflet is also an important mechanism of s...
